What Gets Documented

Every Session
Produces Evidence.

Attendance counts, community themes, referrals made, warm handoffs completed, and accompaniment that followed. At pilot completion, sponsors receive a full outcome report — the documented case for the fall institutional contract.

Attendance Data

Total people in the room and those who engaged in conversation — per session.

Community Themes

2–3 themes that came up organically in each session — the room talking, not the facilitator.

Referral Pathways

Every connection made — organization, type of support, how they were connected.

Warm Handoffs & Accompaniment

Direct human-to-human referrals made. When someone was walked in, not just pointed toward the door.

Session Recap

Narrative summary of what happened, what was said, and what it means for the community.

Full Outcome Report

Delivered to all sponsors at pilot completion — the complete proof of concept for the fall contract.

After The Pilot

The Fall Contract.

The four-session pilot becomes the documented proof of concept for a six-session fall institutional contract — targeted at Boston Public Health Commission, RIZE Massachusetts, and community health center partners.

Pilot sponsors are positioned as co-architects of a model that institutions have been trying to build for years. The investment in the pilot is what makes the fall contract possible.
